MISSION:

The mission of Creative Solutions to Independence (C.S.I.) is to provide residential options for individuals with intellectual disabilities and physical disabilities that maximize their ability to exercise their choices and preferences in achieving as much independence as each is individually capable.  C.S.I. provides supports that allow individuals to live in the residence of their choosing; that protect the individual’s rights, independence, and safety; that facilitate the increase of skills, competencies and opportunities to make choices and that enable the individual to maintain and strengthen valued family and other personal relationships.

 

It is the major focus of C.S.I. service and support efforts to assist Individuals to participate in their own communities to the degree desired.  In accomplishing this, CSI helps to facilitate opportunities for Individuals to interact and develop relationships with persons who are not disabled.

LEADERSHIP:

Kimberly Wells, CEO

Kimberly Wells is owner and founder of Creative Solutions to Independence/Day to Day Independence founded in 2008.  Kimberly has over twenty five years of experience working with individuals with developmental disabilities and mental illness.  

 

Previously, Kimberly was a Program Coordinator for a large residential provider for 10 years.  Kimberly was member of Behavior Support Committee for Lake County Department of Developmental Disabilities.

STAFF BACKGROUND CHECKS:

Once an employee is selected to be part of the C.S.I. Team they must then be able to pass the following background checks:

  • Ohio Drivers

  • Ohio Criminal Bureau of Criminal Investigation (BCI)

  • Ohio Nurse Aid Registry

  • Ohio Department of Medicaid Provider Exclusion and Suspension List

  • Inspector General Database

  • Drug Testing

  • MR/DD Abuser Registry

  • FBI (If have not been a resident of Ohio for the last five years)

  • Sex Offender & Child-Victim Offender Database

  • US Award Management Database

  • DRC Offender Database

  • 2 Personal References

  • 1 Past Employer Reference
